Outcomes and Evaluation Platform

The PCI has partnered with Know Your Own Health to make an outcomes and evaluation platform available to practices and PCNs, helping you evidence impact, improve delivery, and report consistently across your population.

The platform has been designed specifically for NHS Health and Wellbeing Coaches and enables NHS commissioners and PCNs to measure, compare and evidence the impact of Health and Wellbeing Coaching using a single, standardised outcomes framework. In use for NHS-commissioned services since 2013, it supports consistent reporting at PCN and system level, demonstrating impact on patient activation, patient outcomes and service use.

The platform supports any coaching approach with free built-in outcome measures and the ability to add additional questionnaires including licensed questionnaires such as the PAM®, where licensed for the locality and subject to any additional fees being met.

  • Purpose-built caseload management system able to support any coaching approach
  • Standardised outcomes across PCNs or other entities (system comparability)
  • Service impact (self-management/activation measures, patient reported outcomes, patient experience, ability to link with clinical service usage data)

  • Patient activation or self-efficacy scores – gauge knowledge, skills and confidence to self-manage
  • Feedback questionnaires – capture patient experience
  • Patient reported outcomes
  • Able to accommodate other measures being used in localities
A Caseload Management System to:
  • Manage referrals to coaching
  • Track progress of all patients through the coaching journey
  • Track referral numbers, type of sessions (e.g. face-to-face), number and duration of sessions, etc.
  • Provide real-time data

Example: The platform provided the dataset used in a recent prospective service evaluation published in BMC Public Health (Springer Nature) of health coaching delivered through Primary Care. This evaluation used the StACC coaching approach; however, the platform can be used to collect the data for any coaching approach.

Cost

Fees
Access to the platform is charged as follows:

GP Network (e.g. PCN, i.e. can be shared between linked practices): £300 pm
Single Practice (if not being shared as part of GP network): £200 pm
Additional Coaches (first coach included in cost): £30 pm

All fees are exclusive of VAT unless stated otherwise

Billing
Fees (charged by full months) are billed quarterly in advance on the following dates:
1 April, 1 July, 1 Oct, 1 Jan

Terms and conditions

  • The contract has a minimum initial term of 6 months.
  • After the minimum term, the contract continues on a rolling basis.
  • Two months’ notice is required to cancel.
  • Access to the platform is provided once you have completed the registration form below, and payment arrangements are in place.
