Over time you will be able to find a Personalised Care Institute accredited course in your area and be confident that the training you access is of the highest standard and follows the personalised care curriculum.

Designed by Coach-Trained Health Care professionals, this two-day programme provides the ideal opportunity for colleagues in health, social and community are to discover and develop health coaching mind-sets and skills tailored to activation. From feedback, our training is experiential, authentic and immediately impactful.

Know Your Own Health has been specialising in Supported Self-Management since 2010, working with other leaders in the field and learning from on-the-ground experience of how supported self-management works and how to achieve the best outcomes for patients and the NHS. Our expertise combines face-to-face health coaching and digital communications.

  • 2-day ‘Workforce Training’ for Link Workers, Care Coordinators and other Health, Social and Community Care workers.

Know Your Own Health’s ‘Better Conversations’ workforce training programme complements KYOH’s Health & Wellbeing Coach Training. Also built on self-management principles, as contained in the PCI’s health coaching core curriculum, the training enables health, social and community care providers to develop the skills to support patient self-management and personalised care.

The Know Your Own Health and Wellbeing Coach training programme is built on Supported Self-Management Programme principles. Support is tailored to the individual and focuses on patient ownership, or ‘activation’. This is a skilled role and KYOH’s training and supervision programme includes ongoing skills development to achieve reliable high-quality outcomes.


DNA Insight

DNA Insight is a Health and Care training consultancy. focusing on providing training and consultancy advice to Primary Care organisations and ICSs/ICPs in support of the Personalised Care agenda of the NHS Long Term Plan. Our courses set you up for success, and have a reputation for being engaging, practical and enjoyable.

SocialPrescriberPlus is for Social Prescribers, Health Coaches and Care Coordinators operating across the Health and Care sectors. Highly practical in nature, the three half-day workshops provide the skills, knowledge and confidence to help you make an immediate impact. Incorporating the key skills of Active Listening, Motivational Interviewing and Health Coaching, SocialPrescriberPlus can be delivered face to face or online.
