Know Your Own Health’s ‘Better Conversations’ workforce training programme complements KYOH’s Health & Wellbeing Coach Training. Also built on self-management principles, as contained in the PCI’s health coaching core curriculum, the training enables health, social and community care providers to develop the skills to support patient self-management and personalised care.

The Know Your Own Health and Wellbeing Coach training programme is built on Supported Self-Management Programme principles. Support is tailored to the individual and focuses on patient ownership, or ‘activation’. This is a skilled role and KYOH’s training and supervision programme includes ongoing skills development to achieve reliable high-quality outcomes.

Bridges Self-Management

Bridges Self-Management is a social enterprise that exists to make a difference to the lives of people living with acute and long-term conditions. Working with healthcare teams since 2013, Bridges fosters sustainable cultural change by creating a shared ethos of self-management support which fits with a team’s language, systems and processes.

The Bridges model is underpinned by the principles of social cognitive theory and the concept of self-efficacy. Our evidenced based programme uses a 5-stage approach to sustainable system change and is directed towards whole teams across the patient pathway. All of our workshops are co-delivered by clinicians and people who live with a long-term condition.

Practice Managers Association

As a membership organisation for primary care, PMA provide training, education and support to non-clinical workers. We work closely with NHS England, NHS Digital, NHSx, HEE, RCGP, Training Hubs and others. PMA offerings include workshops, web workshops, apprenticeship standards, diplomas, e-learning programmes, webinars, podcasts, consultancy and conference management services.
